Please complete all fields below. ALL volunteers must complete this form and submit documents by 5 PM the Tuesday of the week your group is volunteering in order to be eligible.
- All scanned documents must be submitted prior to the organization you are volunteering with being able to sign a contract.
- All Volunteers MUST submit the following:
- Valid US Driver’s License (must have had for at least one year)
- Valid Student ID
- Please Note: Unfortunately, we cannot accept driver’s licenses that say “Provisional” on the physical card, please either obtain a license from your state without this noted or obtain a Colorado license.
The best way to submit your documents is to take one clear image with the front of both your Driver’s License and your RamCard. We do not need to see the back of either card.
Important: If you don’t receive a confirmation email shortly after submitting your documents, your submission may not have gone through. Please fill out the form again to ensure it's received.
